Initial Appointment
     (history, exam and treatment)   $120

Standard Appointments
      (20-30 minutes)    $50

Extended Appointments:
       (30-45 minutes)   $75
       (45-60 minutes)  $100

PAYMENT POLICY

• Payment is due at the time of each appointment.
• Cash, personal check, debit card and all major credit cards are accepted.
• Youngsters
  • Included in their parents appointment at no additional charge as long as all adjustments are completed within the standard appointment time.
  • When extra time is needed to complete with all family members, the charge is $75 for up to 45 minutes and $100 for up to 60 minutes.
• This office does not participate in any HMOs or PPOs.
• For those with health insurance benefits, a receipt is provided for you to submit to your insurance provider.
• Workman's Compensation claims are not accepted.
• Personal Injury claims are occasionally accepted on a case by case basis.
